Vasopressin is synthesized in the hypothalamus and released in conditions such as hyperosmolality, hypotension and hypovolemia.

Vasopressin is not actually a vasopressor medication. It is antidiuretic hormone (ADH), which is a vasoconstrictor without the inotopic or chronotropic effects. It acts on the kidneys and blood vessels and retains fluid within the body. By increasing the fluid retained, this subsequently increases the patient’s blood pressure.

Transitioning to a different medical center, I saw more cardiac anesthesiologists and intensivists titrating vasopressin anywhere from 0.01 to 0.10 units/minute. GI hemorrhage What are the Actions of Vasopressin (Pitressin)? Increases water permeability of the kidney’s collecting duct and distal convoluted tubule leading to water retention, also increases peripheral vascular resistance leading to increased BP What is the Therapeutic Class of Vasopressin (Pitressin)? Norepinephrine remains the most commonly used vasopressor for the treatment of hypotension in septic shock.

Vasopressin may preferentially cause vasoconstriction of post-glomerular arterioles in the kidney, causing improvement in renal function. It may cause some pulmonary vasodilation, which can be helpful in the context of pulmonary hypertension. Vasopressin shouldn't generally be given peripherally (if it extravasates, there is no antidote).

Vasopressin, also known as antidiuretic hormone, is released within the body in response to hypotension or hypovolemia. 15 In septic shock, vasopressin levels are elevated, then become depleted by 96 hours, with one study identifying 32% of patients having relative vasopressin deficiency (≤ 3.6 pg/mL). 16 Vasopressin causes vascular smooth muscle constriction through binding to V1 receptors Because vasopressin is a potent vasopressor, infusions of vasopressin in patients with several forms of shock have led to improved organ perfusion, increased mean arterial pressure, improved blood pressure and neurologic function. 2,5,6 Several studies have suggested that low-dose vasopressin (<0.04 units/min) is safe and effective for the treatment of vasodilatory shock. 1,2,5 It is also Summary: There is growing evidence that vasopressin infusion in septic shock is safe and effective. Several studies published this year support the hypothesis that vasopressin should be used as a continuous low-dose infusion (between 0.01 and 0.04 U/min in adults) and not titrated as a single vasopressor agent. So, normally, we all have vasopressin or anti-diuretic hormone present in our body but the level is not that high that it will cause vasoconstriction.

This precursor circulates along axons of magnocellular neurons, through the pituitary stalk, to the post-pituitary gland. Vasopressin is stored in the post-pituitary gland, mostly in the intracellular compartment. Vasopressin levels ≥ 20 pg.ml −1 cause maximal increase in urine osmolality. The half‐life of vasopressin is 10–35 min. It is rapidly metabolised by liver and kidney vasopressinases . Vasopressin receptors.
